A total of 7,921 participants were enrolled in this study at a tertiary care hospital in the United States in 2015. Participants underwent a comprehensive clinical assessment, including a comprehensive blood glucose assessment (BGA), physical examination, and laboratory test results. All participants were asked to complete a questionnaire (10 questions) to determine their health status and to give their medications. Participants were invited to a medical evaluation and asked to complete a questionnaire and receive a referral letter.
